The Story of Panch mukhi Hanuman
According to Hindu texts Hanuman ji took this five faced form during the battle of Lord Rama and the demon king Ravana. During the time of the war Ravana took help from his brother Ahiravan, the ruler of Patal Lok (the underworld) to kill Lord Rama. Ahiravan who was a powerful sorcerer and great devotee of Goddess Bhavani kidnapped Lord Rama and Lakshmana while they were asleep and took them to his patal lok for their sacrifice.
Ahiravan lit five divine lamps in five different directions. He had a boon which was given to him by Goddess Bhavani and the boon was that unless all five lamps were extinguished at the same time no one would be able to defeat him and because of this Lord Hanuman transformed himself into his Panchmukhi (five-faced) form so that he can extinguish each lamp facing a different direction. He not only extinguished the lamps but he also killed Ahiravan and saved the life of Lord Rama and his brother Lakshmana.
Since then people started worshipping Panch mukhi Hanuman as a symbol of protection, intelligence, and victory over evil.
